How to Register

Patients wishing to register with our practice should approach our receptionists who will guide then through the process of registration.

Since April 2004 the local Primary Care Organisation has advised that practices should define a 'practice boundary' for patient acceptances.

For further information on our practice area please ask our receptionists.

Patient registrations will be accepted for :-


  •   All patients living in or moving into the defined practice area.
  •   Patients will be asked to provide formal proof of address with their registration form.


If the address is not within the practice area, you will be advised to register with a practice nearer to where you live and given information regarding services available or directed to NHS Choices to search for local services

  • Children of existing patients.
  • Patients wishing to transfer from another practice but not moving address will be encouraged to stay with their current practice, or contact NHS Mid Essex
  • If patients fulfill the above criteria then they will be asked to complete registration documents.
